West Covina, CA. – 72-year-old Jose Lopez Centeno died in a chain-reaction crash that happened early Saturday morning in West Covina, the Los Angeles County Medical Examiner's Office said.
A woman was also hospitalized after the accident that happened around 4 a.m. on the eastbound San Bernardino (10) Freeway near Azusa Avenue.
According to the California Highway Patrol, Centeno, a resident of Glendale, was driving a 2012 Honda Civic in the fast lane when he was rear-ended by a 2011 Toyota Camry driven by a 20-year-old Pomona man under unclear circumstances.
The force of the impact sent the Civic to the far right lane, where it collided with a 2023 Kenworth big rig driven by a 64-year-old Carson man.
The Civic became disabled in traffic lanes and was struck by a 2021 Mazda 3 driven by a 19-year-old Montclair woman.
Centeno was pronounced dead at the scene.
A passenger in the Mazda, only identified as a 19-year-old woman from Pomona, was taken to the hospital with moderate injuries.
An investigation into the crash is ongoing.

According to the California Highway Patrol, the accident happened around 4 a.m. on the San Bernardino (10) Freeway at Azusa Avenue.
