The incident took place on Healdsburg Avenue and Parkland Farms Boulevard at around 12:19 p.m. September 26.
Police said Glenn Merrifield, 50, was riding a motorcycle that collided with a vehicle in the area.
Merrifield was taken to a hospital with major injuries.
The driver of the other vehicle was treated for moderate injuries.
Police said Merrifield was arrested for DUI causing injury and possession of a controlled substance.
The accident is under investigation.
