, AZ.- The man who was killed in a suspected DUI accident early Sunday morning in west Phoenix has been identified as Juan Andres Ortiz, 38, according to the Phoenix Police Department.

The crash happened near 67th and Virginia avenues at about 3:15 a.m. November 23.

According to a preliminary investigation, a man was driving northbound on 67th Avenue at a high rate of speed when he collided with a southbound vehicle making a left turn onto Virginia Avenue.

Authorities said the northbound vehicle did not have its headlights on.

Ortiz was the driver of the southbound car and was declared deceased at the scene

Two other people were taken to a hospital as a precaution.

Authorities said the driver of the northbound vehicle, Andy Lepe, 19, was driving under the influence of drugs or alcohol.

Lepe was arrested and booked on charges including aggravated assault, manslaughter, and endangerment.

The investigation is ongoing.
