Coos County, OR – One person was killed, and another was arrested following a deadly wrong-way, head-on collision on Highway 540 in Coos County, according to the Oregon State Police (OSP).
The fatal crash occurred at approximately 7:35 p.m. Sunday, October 4, 2026, near milepost 5 on Highway 540.
A silver Chevrolet Aveo was traveling eastbound in the westbound lane when it collided head-on with a white Toyota Tacoma traveling in the opposite direction.
The Chevrolet driver, 41-year-old George Cole Cleveland of Coos Bay, was pronounced dead at the scene. The Toyota driver, 35-year-old Aric Dwayne Allen Adkins of Coos Bay, suffered minor injuries.
During the investigation, Adkins was arrested on suspicion of DUII.
The crash affected Highway 540 for about three hours while the scene investigation was conducted. The investigation remains ongoing
