Ontario, CA – The authorities have released the identity of a person who died after two vehicles collided on the 15 Freeway in Ontario early Tuesday.
The San Bernardino County Medical Examiner identified the victim as Ismael Ramirez, a 52-year-old resident of Ontario.
Ramirez was pronounced dead at the scene after a two-vehicle collision that happened at about 4:47 a.m. on April 28, 2026, on southbound I-15 just north of Jurupa Avenue.

Ontario, CA – One person was killed in a two-vehicle crash on Interstate 15 in Ontario early Tuesday morning, according to the California Highway Patrol.
The fatal collision occurred on the southbound I-15 just north of Jurupa Avenue at approximately 4:47 a.m.
According to the CHP, two vehicles, including a white sedan and a work truck, were involved in the crash. The collision left the vehicles blocking multiple lanes, with only the far-right lane initially open to traffic.
One individual was found partially ejected from a vehicle and was later pronounced dead at the scene at approximately 5:06 a.m. The name of the victim was not immediately released. Additional injuries have not been confirmed. No arrests have been reported.
Emergency responders, including fire personnel and paramedics, arrived at the scene and took over CPR efforts before the victim was pronounced deceased. Authorities also requested the coroner to respond.
The crash prompted a SigAlert at approximately 5:06 a.m., with lanes 1, 2, and 3 closed and traffic heavily impacted in the area. A hard closure was initiated, and traffic was diverted as crews worked to clear the scene.
The incident remains under investigation by the California Highway Patrol.
