Surprise, AZ – The driver who was killed in a two-vehicle collision at an intersection in Surprise on Friday morning has been identified as Janice Livesay, 65.
Police said the incident happened when a vehicle heading westbound on Waddell Road attempted to turn left onto Sarival Avenue and collided with a vehicle heading eastbound on Waddell.
Both drivers were taken to the hospital, where one of them, later identified as Livesay, succumbed to her injuries.

Surprise, AZ – One person was killed and another was injured in a crash involving at least two vehicles in Surprise on Friday morning, according to police.
The deadly crash happened just before 11:00 a.m. on October 31 at the intersection of Waddell Road and Sarival Avenue.
The police said both drivers were injured and transported to a hospital for treatment.
One of the drivers succumbed to their injuries at the hospital.
At least two vehicles, a sedan and a red Tesla, were involved in the crash.
The sedan sustained heavy damage on its passenger side, while the Tesla's front bumper was ripped off.
No additional information regarding the crash was immediately available.
The crash remains under investigation.
